Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, interviews, reviewed and collected documentation. It was alleged that staff members did not meet the infant's diapering needs resulting in a rash. Diapering logs revealed infants being changed less than every 2 hours. Interviews with parents did not reveal any disclosures of rashes due to lack of diapering but parents did share a concern about diapering at the end of the day. It was alleged that staff members fed infants expired foods. LPA reviewed Infants Needs and Service plans, menus, and daily food reporting on the app. LPA observed food being served to infants and did not see any expired or old food(s), milk, or formula.

LPA did observe staff members who were unaware of the difference in qualified and not qualified staff members which can impact ratio.
Based on interviews, observations, documentation, and other information gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegation is UNSUBSTANTIATED. In the areas that were evaluated on 09/03/2026, no deficiencies were cited during today's inspection. An exit interview was conducted with Director DiPippo and Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
