It was found that there were multiple days, when the data was not entered correctly into the application. Hence, it was found that facility does not has evidence of changing diapers on time. The evidence of soiled diaper was collected from the reporting party. During record review and interviews, it was found that facility is using 'incident reports' to inform the parents about any injury to the child. LPA observed numerous incident reports, which indicated the quantity of incident happening at this facility. Previously, during the conclusion of a complaint on February 18, 2020, this facility was cited for children’s personal rights and allegation of keeping children in soiled diapers was unsubstantiated. Based on LPAs observations and interviews which were conducted and record review, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.
