On Wednesday 2 September 2026, Licensing Program Analyst (LPA) Fabian Schwartz met with Director Artastia Bolar to Open and Close a complaint investigation. At time of inspection there were 14 infant/toddlers being supervised by 4 staff in 2 rooms.

The department received a complaint alleging that Staff did not administer day care child’s prescribed heart medication and Staff did not notify parent that day care child did not receive the required medication while in care. During today’s inspection, LPA made observations, gathered documents, and conducted interviews. During complaint investigation, LPA observed a medication log for child in care. Medication log was missing 5 dates from the log when child attended care and given medication should have been documented in a log. Interviews confirmed there have been days where child did not receive required medication which is consitent with this log missing dates........Report Continued on LIC9099-C........

On Wednesday 2 September 2026, at approximately 10:15am, Licensing Program Analyst (LPA) Fabian Schwartz met with Director Artastia Bolar to Open and Close a complaint investigation. At time of inspection there were 14 infant/toddlers being supervised by 4 staff in 2 rooms.

It was alleged that Staff falsify licensing records. Throughout the course of the investigation, LPA conducted interviews, reviewed documents, and made observations. Throughout the investigation, LPA could not obtain evidence to meet the threshold to substantiate this allegation.

Although the alleged violation may have happened or is valid, the preponderance of evidence standard has not been met to fully prove or disprove that they did or did not occur, therefore, this allegation is unsubstantiated. 2 other allegations from this complaint were substantiated and explained in further detail on separate pages of this report. An exit interview was conducted and a notice of site visit and appeal rights were provided. Notice of site visit shall remain posted for 30 days.
