13 | On 7/7/2026, License Program Analyst (LPA) Pa Dao Vang conducted a complaint investigation and met with Facility Representative, Lizeth Sanchez Padilla. It was alleged that a day-care child sustained unexplained injuries while in care. LPA observed 33 children supervised by 9 staff members at nap time.
Through the investigation, LPA made observations, conducted interviews, reviewed a file, and gathered documentation. LPA learned that on 2/9/2026, C1 complained about pain on the back to S2 when they were outside. S2 found loop red marks on C1’s back. LPA learned that C1 did not have any accidents at the facility prior to finding the marks. In all the areas inspected, LPA observed no violations. LPA did not learn of any violations from the interviews.
Although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. No title 22 deficiencies are cited. Exit interview was conducted with Facility Representative, Lizeth Sanchez Padilla, and Notice of Site Visit was also provided.
