On Thursday 23 July 2026 at approximately 10:15am, Licensing Program Analyst (LPA) Fabian Schwartz met with Director Samantha Fernandez to open and close a complaint investigation. At time of inspection there were 42 preschool children being supervised by 10 staff in 3 classrooms.

It was alleged that due to lack of supervision, child exited through door into the parking lot. Throughout the course of the investigation, LPA conducted interview, reviewed documents, and made observations. Facility did have a self reported incident which occurred on 16 July 2026 in which 2 children exited door of classroom into parking lot. During investigation, LPA observed footage of incident captured by facility. During this incident, children were supervised by a staff who followed children outside. Although children did make it out of classroom, it was not as a result of a lack of supervision defined by Title 22 regulations. Report Continued on LIC9099-C........

Although the alleged violation may have happened or is valid, the preponderance of evidence standard has not been met to fully prove or disprove that it did or did not occur, therefore, the allegation is unsubstantiated. An exit interview was conducted with director and a notice of site visit and appeal rights were provided. Notice of site visit shall remain posted for 30 days.
