13 | Licensing Program Analyst (LPA) Christopher Bello met licensee Deanie Guerin to open and close a complaint investigation, regarding the above allegation. Upon arrival, LPA observed four children. Also present was licensee’s daughter who was acting as an assistance. LPA made observations, gathered documents pertaining to the investigation and conducted interviews. It was alleged that the provider was unaware that four children left the facility and were found in the middle of a Mainstreet that connects to the neighborhood by members of the community. Licensee self-reported the incident.
Licensee stated that she corrected the issue by installing new sensors on the door and camera.
Based on LPA's investigation, the preponderance of evidence standard has been met, therefore, the above allegations are found to be SUBSTANTIATED. A civil penalty of $500 dollars was accessed.
