13 | On April 3, 2023 at 10:30 AM, Licensing Program Analyst (LPA) Tanya Washington met with Licensee, Olena Shevchuk to deliver complaint findings for the allegations above. Upon arrival LPA observed Licensee and her assistant providing care to five daycare children. Also present is Licensee's minor daughter.
During the course of the investigation LPA conducted interviews with Licensee, her assistant, and parents of daycare children. LPA also toured the areas of the home accessible to children, inspected the storage area for medication and made observations.
Reporting Party alleged that a child sustained an injury to their eye at daycare. Staff interviews revealed that two young children were holding a toy car at the same time, one child let go of the toy, causing it to hit the other child near the eye which resulted in a small mark under the eye. Licensee stated, she observed the brief encounter with two children and attempted to redirect the children and encouraged the children to share. Licensee stated, the child did not cry when the toy hit them in the eye and the mark under the eye appeared after nap time. Licensee’s assistant informed the child’s parent via text message of what happened when the child woke up.
