13 | Licensing Program Analyst (LPA) Jennifer Velasco met with Facility Representative, Licensee Bonnie Roninger, for the purpose of conducting an unannounced initia complaint investigation inspection pertaining to the above allegations. The purpose of today's inspection was explained to the licensee. During this investigation, LPA conducted interviews, observed care, and obtained relevant documentation. Though there was evidence of a rodent infestation in the off-limits garage of the home, witness statements and LPA review of documents demonstrated the licensee removed this infestation. Licensee provided documentation of timely and ongoing use of exterminator services and removal of potential harboring items such as an ivy patch, perishable food, and clutter in the garage. This allegation is unsubstantiated. Witness statements and document review also showed the licensee is in the home during hours of care for at least the required 80% of time the facility is operating. Although the allegation(s) may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is unsubstantiated. Exit interview was conducted and report was reviewed with Facility Representative, Bonnie Roninger. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days. |
