On 8/20/2026, License Program Analyst (LPA) Pa Dao Vang conducted a complaint investigation and met with Director Saira Singh and Licensee Abrica Castaneda. It was alleged that the staff did not ensure food was cut up/safe for children to eat. There are 8 infants, 17 toddlers, and 21 preschoolers supervised by 10 staff members.

Throughout the investigation, LPA made observations, conducted an interview, reviewed files, and gathered documentation. In the infant room, LPA observed the staff feeding long spaghetti noodles, orange slices with the rind (2 inches in length), and green beans (about 1 ½ to 2 inches in length). LPA observed 2 infants struggling to eat their spaghetti noodles for several minutes. One of the infants was able to put the whole noddle in their mouth and pulled it out again.

Through the interviews, LPA learned that during the 4th of July barbecue event on 7/2/2026, the infants were served half a hot dog with half a bun, corn on a cob, chopped watermelon, milk, and other snacks. LPA also learned that parents that attended the event fed their own children. There were several infants that their parents couldn’t attend event and the staff members fed them the same food. LPA also reviewed the communication log through the app and observed images of infants being served the same foods. The Director stated that most of the infants are starting on soiled foods. Director also stated that there was no formal menu for the event, but they sent a message through the app about the foods on that day. There are 7 out 8 staff members and 4 out 4 parents confirmed that the infants were fed half a hot dog with a half a bun during the event. Some staff members stated that they weren't able to access the scissors to cut the hot dog up for the infants to eat.

Based on observations and interviews, the preponderance of evidence results in substantiating the allegation a facility staff did not ensure food was cut up/safe for children to eat. This posed as a risk to the daycare children while in care. The Title 22 deficiency is being cited on LIC 9099-D pages. An exit interview, appeal rights and a notice of site visit were provided to Director Saira Singh. A Notice of Site Visit posted. Failure to comply with posting requirements can result in a $100 penalty.
