On 7/30/2026, License Program Analyst (LPA) Pa Dao Vang conducted a complaint investigation and met with Director Saira Singh and Licensee Abrica Castaneda. It was alleged that the staff are operating out of ratio. There are 8 infants, 17 toddlers, and 21 preschoolers supervised by 10 staff members.

Throughout the investigation, LPA made observations, conducted an interview, reviewed files, and gathered documentation. LPA observed children being dropped off in the mornings as staff show up for their shifts. LPA also reviewed the staff’s work schedules and the daily activities for the classrooms.

Through the staff interviews, LPA learned that the infant classroom has operated out of ratio with 2 staff members supervising 9 infants around 8:00 AM to 8:30 AM. LPA learned that the the infant room will be out of ratio until another staff member start their shift. Director stated that S3 and S4 mentioned about being over ratio. Director also specified that the facility is currently hiring additional staff and adjusted the staff's work schedules to make sure the facitliy stay in complaince with the ratio.

On 7/30/2026, License Program Analyst (LPA) Pa Dao Vang conducted a complaint investigation and met with Director Saira Singh and Licensee Abrica Castaneda. It was alleged that the staff left a child soiled and staff are not ensuring to provide adequate logs for children in care. There are 8 infants, 17 toddlers, and 21 preschoolers supervised by 10 staff members.

Throughout the investigation, LPA made observations, conducted an interview, reviewed files, and gathered documentation. LPAs also observed communication logging through the app. LPA observed the staff members changing children’s diapers every 2 hours and logging into the app to notify the parents. LPA conducted 8 out of 8 staff members’ and 3 out of 4 parents’ interviews, in which revealed no concerns regarding the above allegations. In all the areas inspected during the investigation, LPA observed no violations and did not learn of any from the interview.

Director and Licensee stated that the staff are starting their shifts 15 minutes eariler. They are currently firing additional staff and will hold off on enrollments to stay in complaince with the ratio.

There are 6 out of 8 staff members and 2 out of 4 parents confirming that the infant classroom have been out of ratio a couple of times. A staff member also stated that the toddler classroom was out of ratio during nap time with 2 staff members supervising more than 13 toddlers. The parents’ interviews revealed that they’ve heard the staff calling on the walkie-talkie about being out of ratio at drop-offs. They also seen the messages sent through the app requesting additional staff for ratio.

Based on observations and interviews, the preponderance of evidence results in substantiating the allegation a facility staff are operating out of ratio. This posed as a risk to the daycare children while in care.

The Title 22 deficiency is being cited on LIC 9099-D pages. Director acknowledges, that for TYPE A DEFICIENCIES ONLY upon receipt, facility shall post LIC 9099-D with Type A deficiency for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the Licensee. LIC9224 and Appeal Rights were provided. An exit interview, appeal rights and a notice of site visit were provided to Director Saira Singh. A Notice of Site Visit posted. Failure to comply with posting requirements can result in a $100 penalty.
