On 01/23/2023, Licensing Program Analyst (LPA) Kay Turner arrived at Canfield Family Child Care to conclude the investigation regarding the above allegation. LPA conducted a tour of the facility and the census was taken. During the investigation interviews were conducted with pertinent parties and documentation was collected.

On October 24, 2022, a complaint was received alleging the facility was violating personal rights. The complaint stated the licensee isolated an infant while in care. During the course of the investigation, the licensee disclosed to isolating an infant in a room alone for at least 15 minutes and reported to sitting directly outside of the doorway. However, upon reviewing information from documentation collected, the licensee not only states the child was separated from the other children at the facility, but also to leaving the child alone resulting in the infant screaming once the licensee exited the area. The licensee explained the child’s isolation in the room was due to incidents of biting/attempted biting of the other children in care at the facility.

The room was observed to be a smaller room adjacent to the living room that was set up with clean, safe and appropriate toys. There is a safety gate at the door. The licensee has the ability to sit outside the doorway and observe children in the room and children in the living room of the family child care home. The LPA was unable to ascertain the number of instances this occurred within the facility, but based on documentation collected it was on more than one instance. LPA Turner informed the licensee that each child must provided healthful and comfortable accommodations while in care.

Based on all the information obtained from pertinent parties, documentation, records review during inspection, the department has determined the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

See LIC 9099-D for deficiencies.

Exit interview was conducted with Haley Canfield, licensee. A copy of this report, Notice of Site Visit, and Appeal Rights were provided A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

On 01/23/2023, Licensing Program Analyst (LPA) Kay Turner arrived at the family child care home to conclude the investigation regarding the above allegations. LPA conducted a tour of the home and the census was taken. During the investigation interviews were conducted with pertinent parties and documentation was collected.

On October 24, 2022, a complaint was received alleging there was a lack of care/supervision. The complaint stated an infant sustained unexplained injury while in care. Review of documentation indicated that while an infant possibly did sustain minor scratches while in care at the home, the infant’s authorized representative/parent spoke with the licensee approximately one week later. The licensee was unaware of any incidents that would present the formation of injuries. However, the licensee agreed to pay closer attention.

Based on the interviews conducted, the review of the pertinent documentation coupled with the time lapse of the injuries, the allegations are UNSUBSTANTIATED. A finding that the allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the allegations occurred.

No deficiencies were found at this time.

The report was reviewed and an exit interview was conducted with the licensee, Haley Canfield. A Notice of Site Visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. A copy of this report was provided to the licensee.
