On the date and time listed above, Licensing Program Analyst (LPA) Giselle Carbullido conducted a subsequent complaint investigation to deliver final findings. An initial visit was conducted on 09/23/26 at which time LPA conducted interviews and reviewed records. LPA met with Christy Avila, Licensee/Jocelyn Alvarez, Director, toured the facility and took a census.
During the investigation, LPA reviewed records, obtained photos and interviewed pertinent parties, including facility staff and children.
It was alleged that facility staff did not take action to keep the center free of insects specifically, there are conditions that contribute to the breeding of pests that bite children. Staff interviews stated mosquitos are indoors and outdoors, and children are bit repeatedly despite use of insect repelling patches and bracelets from home. Interviews disclosed pest control is used, however there is daily standing water on the premises
which contributes to breeding of pests. Interviews also stated staff are bit as well and parents have expressed concern for their children. Children interviews stated they are bit, have itchy bumps and pointed to scars from past and current bites on their limbs and stomach area.

During tour of the facility at 11AM, LPA observed flying insects in classrooms, church hall and outdoors. Additionally, at 11:10AM, LPA observed standing areas of water on the outdoor playground with breeding insects.
Record review revealed pest control from Terminex. LPA reviewed a sampling of inspection reports dated Feb. - Sept. 2026. Inspection reports from June -Sept. 2026 note there are ‘open conditions’ that contribute to pest entry into building and/or breeding of pests.
Although the licensee took some measures to keep the center free of insects, there are other areas where measures weren’t fully implemented to address the center and grounds from insects that are a health and safety risk to the children in care.

Based on information gathered from staff interviews and records, the preponderance of evidence standard has been met, therefore, the above allegations are found to be SUBSTANTIATED. Per California Code of Regulations, Title 22, Division 12, CCR 101238.2(d)(1) is cited on the attached LIC9099D.
Appeal rights issued and discussed with facility representative and their signature on this form acknowledges receipt of these rights.

An exit interview was conducted and a copy of this report and notice of site visit were provided to facility representatives, Christy Avila, Licensee/Jocelyn Alvarez, Director. A copy of this report must be made available to the public upon request for three years
