32 | On 09/10/2026, LPA made an unannounced visit to the facility to initiate this complaint investigation. During this visit, LPA toured the facility and conducted interviews with the Licensee and pertinent individuals. LPA recorded corroborating statements from the Licensee and multiple staff which support the facility’s school-age program has been operating at a 15:1, child to teacher ratio. It was stated this ratio 15:1, was believed to be correct ratio for the school age program and was communicated to staff as the maximum number of children a single staff member could supervise on their own.
Regarding unqualified staff supervising children in care, LPA was informed the facility does have a dedicated kitchen staff member whose primary role at the facility is food preparations. It was disclosed by Licensee and staff alike, that on infrequent occurrences, this subject staff member has been utilized to supervise school-age children without the presence of a qualified teacher for brief periods of time, ranging from 10-15 minutes.
As part of this investigation, LPA reviewed pertinent facility records/documents and determined the subject kitchen staff member did not meet the qualifications or educational requirements to supervise groups of children in the absence of a fully qualified staff member. Additionally, LPA discussed Title 22 regulation 101516.3(b)(1) Teacher-Child Ratio, with Licensee and staff, which disclosed, A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children.
Therefore, based on LPA's observations, interviews conducted and records reviewed, the preponderance of evidence standard has been met. The above listed allegations: Facility is operating out of ratio, and unqualified staff are supervising children in care, are found to be SUBSTANTIATED.
Please see attached LIC9099D for Type B deficiencies cited.
A NOTICE OF SITE VISIT WAS GIVEN. LICENSEE WAS INSTRUCTED TO POST IT IN A PROMINENT LOCATION AT THE FACILITY. THE LICENSEE UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS.
An exit interview was conducted, A copy of this report and appeal rights were given to Licensee, Carmen Webb on 09/17/2026.
