This complaint investigation report dated 08/05/2026 was amended to correct information on the citation page. On 08/05/2026, Licensing Program Analyst (LPA) Hanna Cha met with facility representative, Latashia Kelly. The purpose of the visit was to deliver findings for the above allegation. LPA arrived during nap time. LPA observed three teachers providing care and supervision to seven toddlers and 17 preschool-age children.

The investigation consisted of observations and interviews with the Reporting Party (RP), children, staff members, parents, and other relevant parties.

Concerning the allegation that staff do not keep the facility free from odor, interviews conducted revealed that the B Wing classroom had an odor. During the initial visit conducted on 07/21/2026, LPA observed an odor in the B Wing classroom. There were no children present at the time the odor was discovered. The children were relocated and combined with the A Wing classroom. LPA verified that the facility was in compliance with capacity requirements.

LPA observed a Pest Control Specialist arrive to assess the B Wing classroom. Per facility representative, work orders were placed to eliminate the odor. Per parent interviews, no concerns disclosed regarding the odor in the B Wing classroom.

During the visit conducted on 08/05/2026, LPA verified that the odor was eliminated from the B Wing classroom. Per facility representative, the ceiling was cleaned and ceiling tiles were replaced. LPA observed maintenance staff conducting ceiling work at time of visit. The facility hired a contractor. A mold test was completed and all results came back within normal levels. All repairs have been completed.
