13 | On 9/3/2026, Licensing Program Analysts (LPAs) Saraliz Velando and Paul Cortez conducted an unannounced visit to deliver findings regarding the above referenced allegations.
During the investigation, the LPA(s) interviewed relevant parties and reviewed pertinent documentation. Information obtained corroborated that, in early 2024, a staff member used a sleep mat to push down on Child 1 at naptime. No official documentation was available identifying the specific date of the incident. Additionally, the incident was not communicated to the parent in a timely manner.
Further, on 4/19/2024, a child exited the facility unsupervised and remained outside for several minutes. Based on the preponderance of the evidence, all allegations were determined to be substantiated. In accordance with the California Code of Regulations (Title 22, Division 12, Chapter 1), two of the deficiencies are cited on the attached LIC 9099D.
During a case management visit on 5/3/2024, the facility was issued a Type A deficiency, and a $500 civil penalty was imposed for the incident that occurred on 4/19/2024. Therefore, a deficiency for that allegation will not be issued today.
A Notice of Site Visit was provided, and the LPA(s) observed it posted. The Licensee was advised that the notice must remain posted for 30 days. An exit interview was conducted, and a copy of this report and Appeal Rights were discussed and provided. The signature at the bottom of this report confirms receipt.
