25 | Licensing Program Analyst (LPA) Jennifer Lott conducted an announced Case Management visit. LPA was greeted at the front door by Licensee, Magela Maltarollo and granted entry after identifying herself and disclosing the reason for her visit. The purpose of LPA's visit was to review the request for an increase in capacity from eight (8) to fourteen (14) as well as a change to the facility floor plan.
During today’s visit the LPA conducted a tour of the facility with the licensee. Changes to the facility sketch included the use of the living room to the day care classroom and the former classroom to a bedroom. All other rooms in the home remain the same. The following areas will be used for day care; day care classroom, backyard and bathroom #1. Off limit areas include bedroom #1-3 and bathroom #2. LPA found the physical plant is consistent with the newly submitted facility sketch/floor plan. In addition, the Fire Marshall granted a fire clearance for a capacity of fourteen (14) on November 15, 2021. Licensee also meets the COVID-19 posting requirements.
Based on today’s inspection, no deficiencies are being cited as the facility has been found to be in compliance. A license for a capacity of fourteen (14) will be granted pending final file review.
An exit interview was conducted and a copy of this report (LIC 809), Appeal and Licensee Rights (LIC 9058) and Notice of Site Visit (LIC 9213) was provided to Licensee, Magella Maltarollo. The Notice of Site Visit (LIC 9213) must remain posted for thirty (30) days. |
