On 6/27/23 at 10:55am Licensing Program Analyst (LPA) Annette Sutherland conducted an unannounced annual inspection with the Licensee. Upon arrival, LPA met with Licensee Adila Faizi. Also, in the home was Licensee’s husband Mohamed (who helped translate to Dari) and 5 minor children. Licensee was provided the Inspection Checklist (LIC 126). The 1 story home was toured and inspected to ensure an environment safe for the care and supervision of children. There were no day care children in care. Licensee has provided adequate space for the children to eat, sleep and play within the home. Areas used for childcare include Living room, dining room, family room, nook area, bathroom #1 and bedroom #1. Off limits areas include bedroom #2 and bedroom #3 and bathroom #2 and are inaccessible through use of doorknob covers. The licensee has sufficient toys and equipment available. The home has a fenced backyard although is off-limits. There is a nearby park that Licensee takes the children to for outdoor activities. Licensee understands that supervision is always required during outdoor activities. LPA conducted childcare quality management interview with the Licensee. The fireplace is screened.

The fire extinguisher does not meet regulation and is not operational. The carbon monoxide detector/smoke detector combination meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There are no bodies of water on the property. Licensee states that there are no weapons in the home. Licensee has required immunizations. CPR has expired as of March 2023. Licensee is exempt from Mandated Reporter Training as their primary language is Dari. Children’s and Staff records were reviewed and found to be in order.
