32 | Regarding the allegation that the facility did not provide a safe environment, S1 confirmed that, upon arriving at 6:45a on Monday to open the facility, S1 observed two to three displaced ceiling tiles and water on the floor and immediately contacted the Director. S1 stated that the amount of water was a small puddle and was cleaned using several rags and a mop. The general contractor arrived within approximately 30 minutes, followed by Asset Property Management, to assess the plumbing and determine the source of the water intrusion. Director stated that no mold or mildew was found. The ceiling tiles were replaced, and the classroom reopened on 08/26/26. Staff stated that no children or staff became ill because of the incident.
During the inspection, LPA did not observe any missing ceiling tiles, and all classrooms were operational.
Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, therefore the above allegations are found to be Unsubstantiated.
Exit interview conducted and report was reviewed with Assistant Director Cassie Galvan.
A notice of site visit was given and must remain posted for 30 days. LPA observed form posted.
