13 | On 7/29/26 at 8:15 am, Licensing Program Analyst (LPA) Annette Sutherland conducted an unannounced complaint inspection to deliver findings regarding allegation that facility staff improperly handled a child, resulting in injury. LPA met with Director Monique Reycasa. The investigation included interviews with relevant parties and a review of facility records, electronic incident notifications, and other pertinent evidence. The investigation revealed that on June 11, 2026, a child sustained an injury following an incident near the playground. Information obtained indicated that a staff member (S1) assisted the child (C1) after the child became entangled. The child reportedly cried and demonstrated limited use of the affected area. The child subsequently received medical treatment. Although concerns were raised regarding the staff member’s interaction with the child and sustained an injury requiring medical evaluation, the evidence obtained during the investigation was insufficient to determine that staff improperly handled the child. Therefore, the allegation is unsubstantiated. LPA was unable to find corroborating evidence to support the allegation. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, therefore the above allegation is found to be Unsubstantiated. Exit interview conducted and report was reviewed with Director Monique Reycasa. A notice of site visit was given and must remain posted for 30 days
