LPA also observed metal screws were utilized to attach the pieces to the base and some screws were still in their holes. One screw could be seen protruding through the base at the contact point where one of the walls or support pieces would normally be attached. This area were the screw was protruding was identified by Director Castaneda as the area where C1 placed their hand when climbing over the side of the base wall into the inside of the base.
Based on LPA’s observations and interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, are being cited on the attached LIC 9099-D. An exit interview was conducted, and this report was reviewed with and provided to Director Castaneda. Appeal Rights were also discussed and provided. An LIC 9213- Notice of Site Visit was also issued and must remain posted near the main entrance for 30 days. Non-compliance with posting will result in a $100 fine. This report must be accessible to the public for three years.
