13 | On 07/30/2026 Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced complaint inspection in regard to the above allegation. LPA met with Site Director, Beth Reynolds, and informed her of the reason for the visit. There were 2 infants present with 2 staff at the time of the inspection.
During the investigation, LPA conducted interviews with Director, staff, and daycare parents, inspected and took photos of infant equipment and collected facility records.
During interview, Director stated on June 5, 2026, around 10:20 am, Child 1 (C1) fell out of a low feeding chair due to not being secured in the feeding chair's three-point harness. C1 sustained a bump and bruise on their forehead due to the fall and 911 was called. C1 was transported to Rady Children's where they recieved medical care and was released to return home the same day. Director reported the incident timely to licensing via a LIC 624 (Unusual Incident Report). During interview, Staff 1(S1) stated that at the time of the incident, she was attending to other infants in high chairs, and observed C1 stand up in their feeding chair but was unable to get to C1 in time to prevent them from falling. Interviews with daycare parent provided similar information as provided by Director and staff. LPA inpsected the feeding chair used by C1 and found the chair to be age-appropriate and had no broken or missing parts.
Based on information obtained in interviews, the preponderance of evidance standard is met and the allegation of Staff did not properly secure a daycare child in a highchair is found substantiated. |
