On 2/20/24 at 3:15 Licensing Program Analyst (LPA) Adrian Mangina conducted an unannounced visit for the purpose of delivering findings for the complaint received 1/18/24 regarding the above allegation. There were no children in care at the time of the visit. Licensee stated that the facility has been closed since 12/16/24.

It was alleged that Licensee didn’t provide complete reporting details to parents and Licensee failed to report incident to Licensing that a child was injured at the daycare during a fall. Witnesses reported that on 12/15/23, Licensee Mary Podhorsky fell while holding a child in her arms and sustained an injury that required urgent medical care. Licensees admitted that they failed to inform parents that a child was involved in the fall and that no report was made to Licensing verbally within 24 hours or in writing within 7 days. Although LPA could not prove that the child was injured in the fall, there was a bruise found on the child
