25 | On 3/3/23 Licensing Program Managers (LPM’s), Renesha Askew and Monica Cuddy conducted an Office meeting with Licensee, Shahla Ranjbar and her husband Abdohousain (Joe) Shegarfi. The purpose of this meeting is to discuss Licensee’s recent annual inspection and the Licensee’s demeanor and concerns.
On 2/27/23 during the Annual inspection Licensee had a disagreement with LPA regarding the nature and scope of the citations, the timing of the visit (during nap), and with LPA Mangina conducting the visit/presence in the home. During the visit on 2/27/23 the facility was cited for the following citations:
Type A:
· 102417(g)(5) there were multiple buckets, trash cans and tubs filled with water in an area accessible to children in care in the backyard. - LPM advised the visit report will be amended to remove the citation and a Technical violation is being issued today in it's place as Licensee ensured water was dumped during the visit upon being advised of the issue and the water wasn't immediately accessible but could have been a potential risk. Licensee states will now use living room for daycare and will ensure to make sliding door inaccessible that leads to off-limits backyard area.
As well as multiple Type B citations for not having a current/updated children’s roster, not having children’s files, home being cluttered with boxes and bins piled and on the floors of the playroom and adjacent bathroom, no emergency drills conducted since 12/2020, personnel records being incomplete, and area previously marked off limits being accessible/used for daycare space without prior inspection/approval.
LPM discussed backyard hazard as a low barrier/fence is in place to separate a section of the backyard for the children to use for outside play. Due to the height of the barrier direct visual supervision is required at all times to ensure that off limits areas of the backyard, which is not fully fenced, are not accessible to children. LPMs reviewed current sketches on file with Licensee and Licensee completed and provided an updated sketch to show off limits’ areas during this office meeting. |
