On 04/03/24 at 2:45PM, Licensing Program Analyst (LPA) Luigi Gargaro conducted
an unannounced case management visit to the facility. The purpose of the visit was to deliver a criminal record clearance citation to the licensee due to evidence discovered regarding the violation during a complaint previously filed against the day care.

In testimony analyst received during the investigation, it was determined that the licensee’s spouse, Joshua Mathis, has maintained a prominent presence in the facility when it was operating. Mr. Mathis does not have a criminal record clearance and is not allowed to be in a licensed facility until a completed one has been received. The licensee was cited a type A citation today for the violation on the attached 809D page.

Upon receipt of a type A violation, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.

An exit interview was conducted with the licensee. The licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. Analyst printed a copy of the Notice Of Site Visit today and had licensee place it in her facility notice area before he left the home.
