32 | This is an amended version of the original report created on 6/12/2026.
Regarding the supervision allegation, interviews revealed that C1 was playing alone on the playground when C2 went down a nearby slide. As C2 exited the slide, C1 immediately turned and bit C2 on the back, breaking the skin. Staff stated there were two staff members supervising the playground, and because C1 was playing independently, their attention was directed toward other children. Staff immediately responded to the incident. Staff reported there was no provocation by C2 and no behavioral indicators that C1 was about to bite. Although C1 had a history of biting, prior incidents had not resulted in broken skin or bleeding. Documentation reviewed showed the facility had implemented behavior supports, meetings, and learning plans to address C1's behaviors. Parents interviewed did not express concerns regarding the supervision provided by the facility.
Based on the information obtained during the investigation, there was insufficient evidence to determine that the facility improperly disclosed confidential information regarding C1 or that C2's injury resulted from a lack of supervision. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.
Exit interview conducted and report was reviewed with the facility representative Yanira Molina . A notice of site visit was given and must remain posted for 30 days.
