On 01/28/2025 at 2:00 pm, Licensing Program Analyst (LPA) Gerald Poindexter conducted an unannounced case management inspection. This inspection was conducted to observe corrections to deficiencies cited on 11/21/2024. Observed present today were 6 preschool children with director, Izhar Mohamed, Sahra Ali, teacher, and Rukhsana Mehta, Aide, present. Facility was within ratio and capacity.

Several corrections to deficiencies were made prior to today’s visit. LPA provided an extension for others. Observed corrected today:

1596.871(c)(1)(A) Administration of Child Day Care Licensing – All staff present today were background cleared and associated to the facility.

101161(a) Limitations on Capacity and Ambulatory Status – Children present wee spaced; no co-mingling.

101238(g) Buildings and Grounds – All chemicals were stored inaccessible to children.

101216.1(b) Teacher Qualifications and Duties – Staff present today met education qualifications.

101217(d) Personnel Records – Staff records were available at the facility for LPA review.

101221(a) Child’s Records – All enrolled children had files available for review.

101212(b) Reporting Requirements – Director requirements have been reported and updated.
