Regarding allegation that staff did not follow Child #1 (C1) feeding plan and was fed infant oatmeal cereal without parents permission, based on interviews conducted staff disclosed they feed infants based on each child's Needs & Service Plans. It was also disclosed that parent of C1 had submitted an updated plan but it wasn't fully reviewed by staff and it was assumed that infant oatmeal cereal was OK. Based on record review, LPA observed each Needs & Service Plans submitted for C1 where none indicated infant oatmeal cereal, only puree foods.

Based on the information obtained during this investigation the preponderance of evidence standard has been met. Therefore, the above allegation is found to be SUBSTANTIATED.

An exit interview was conducted, and this report was reviewed with the Interim Director, Bertha Tapia, and a copy was provided. Appeal rights were discussed and provided during the exit interview.

A Notice of Site visit was given, and Interim Director understands that it must remain posted for 30 days.
