On 9/1/2026 at 9:00 a.m. Licensing Program Analyst, Cindy Meier conducted an announced Case Management inspection, requested by applicant. LPA met with Owners Peter Cohen, Darren Solomon, and Director, Jazz Chavez. The purpose of the inspection was discussed with owners and director. Applicants have applied to increase the infant component to 24 infants (ages 0-24 months) in classrooms #1 and #3 and decrease the preschool children to 60 (ages 2 to 5 years old) in classrooms #2 and #4. Hours of operation is Monday to Friday from 8:00am to 5:00pm. Currently the license serves 12 infants, (ages 12-24 months) in classroom #3 and 72 preschool children, (ages 2-5 years old), in classrooms #1, #2, and #4. No children were present during the inspection.

LPA continued to tour the facility and measured indoor activity space for infant classrooms #1 and #3. Infant indoor activity space for classrooms #1 and #3 measured 954.25 square feet, which is sufficient to accommodate the requested capacity of twenty-four (24) infants ages (0-24 months). Indoor activity space for preschool classrooms #2 and #4 measured 2100.65 which is sufficient to accommodate the requested capacity of sixty (60) preschool children ages (2-5 years old). Fire clearance was granted on 8/6/2026.

The infant chairs are made of washable, moisture-resistant material. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing tables are at least 3” high. The changing tables are within arm’s reach of a sink. Diapering sinks should not be used for meal preparation or dishwashing.

There shall be at least one potty chair or toilet for every five potty training infants. LPA observed a total of 3 sinks available for the infant’s use. These are sufficient to accommodate the capacity of twenty (24) infants.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

infants who are able to climb out of cribs. Each crib has its own sheet or covering. The sleeping area for infants in classroom #1 is physically separate from the indoor activity space and has a four feet high, sound-absorbing material barrier, designed to minimize the risk of injury to infants.

All infants (0-24 months) have a Needs and Service Plan that outlines their daily food intake. Bottles and solid food are labeled with the child’s name and date. Formula and any solid food is provided by the parents. Sleep logs are documented for all infants in Classrooms #1 & #3. Storage cubbies are readily available, and room accommodates class size.

There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the Director’s office and staff’s restroom.

LPA observed all hazardous items to be inaccessible to children.
Co-owner stated there are no bodies of water and LPA did not observe any bodies of water during the inspection.
Co-owner stated there are no firearms, other weapons, or ammunition in the faciity.
Co-owner stated they do not transport children.

The facility currently has a fully fenced playground area. The fencing is wrought iron and is four feet high. On 6/2/2025 the facility was granted a playground waiver stating infant and preschool children may utilize the same space, follow rotating schedules, and ensure at no time that infants and preschool children utilize the space at the same time. On 6/2/2025 the facility was also granted a waiver for the preschool children to use the outdoor and indoor gross motor play areas on a scheduled basis due to the square footage not being able to accommodate all 72 children at one time. Waivers were submitted that reflect the new capacities for both infants and preschool children, along with the rotating schedules. Approval is pending.

The surfaces of the outdoor activity spaces and playground equipment are maintained, is in safe condition and is free of hazards. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is no climbing structure on the playground. Drinking water is available via the children’s own water bottles when the children are playing outside. LPA observed all hazardous items on the playground to be inaccessible to children. The area includes a shaded rest area via sail shades and pop-up canopies.
