13 | On September 21, 2026, Licensing Program Manager (LPM) Leung and Licensing Program Analyst (LPA) Van conducted an unannounced inspection to conclude a complaint investigation and deliver the findings to the facility. The investigation addressed allegations that staff did not prevent a child from injuring another child and did not report the incident to the child’s authorized representative.
LPA met with Licensee Jenny Chan. At the time of the inspection, the Licensee and an assistant were supervising 9 children (two infants and seven preschoolers). The facility was operating within its licensed capacity and met all staffing and supervision requirements.
As part of the investigation, LPA conducted interviews and reviewed all relevant records, including communication records. Based on the information obtained, there was insufficient evidence to establish that staff failed to prevent a child from injuring another child or failed to report the incident to the child’s authorized representative. The investigation found that the facility met applicable staffing and supervision requirements. Communication records further showed that the child’s authorized representative was notified of the incident by text message through the facility’s application.
