25 | On January 24th, 2025, at 2:00 PM, Licensing Program Analyst (LPA) David Nguyen met co-licensee Sheree Johnson to conduct a proof-of-correction inspection to verify corrections of a Type-A deficiency cited on January 16th, 2025. The purpose of the proof-of-correction inspection was explained. LPA was granted entrance into daycare facility by co-licensee. There were four (4) children present during today’s proof-of-correction inspection.
On January 16th, 2025, the facility was cited for one (1) Type-A deficiency for not keeping the infants’ play yards free of loose articles during naptime.
During today's inspection LPA toured on-limits areas. The Type-A deficiency cited on January 16th, 2025, for not keeping the infants’ play yards free of loose articles during naptime was cleared on January 24th, 2025. Upon arrival, LPA observed three (3) napping infants in the play yards without loose articles in the infants’ play yards during naptime.
During today’s inspection, co-licensee stated that she wanted to make the backyard and the side yards on-limits. LPA toured the backyard and side yards with co-licensee. LPA inspected the backyard and the side yards, and LPA verified that the backyard and the side yards are safe for daycare children to play during outdoor play times.
An exit interview was conducted with co-licensee, Sheree Johnson. Appeal rights and a Notice of Site Visit were provided. Co-licensee’s signature on this form acknowledged the receipt of the form. A notice of site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
