13 | On 06/11/2026, Licensing Program Analysts (LPAs) Janie Davis and Debra Khashe conducted an unannounced visit to open a complaint investigation for the above allegation. LPA met with facility representative Veeresh Kethari. LPA observed 15 children present. Another 2 children arrived and were turned away during the inspection.
Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, reviewed and collected documentation. It was alleged that the facility is operating out of ratio. Observation revealed the facility was operating beyond capacity.
Additionally, LPA Davis returned to the facilty on 07/01/2026. Based off of CCLD ratios, 2 of the children in a ratio of 14 should be school age. LPA observed 13 children preschool age and younger, and one child age five with no proof of enrollment in school. Based on observation, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. A Type-A deficiency was cited on a subsequent 9099-D page.
