13 | On 9/24/2026, at approximately 2:20PM, Licensing Program Analysts (LPAs) Tse and Alvarado conducted an unannounced complaint investigation at the facility. LPAs met with Licensee, Aileeh Carmen Vilchez, and explained the purpose of the visit.
During the course of the investigation, LPA conducted site observations, interviews with relevant parties, and site observations. During a site visit, LPA Tse did observe that a child was napping on the floor without a mat or a blanket. The preponderance of evidence standard has been met; therefore the above allegation is found to be SUBSTANTIATED.

Appeal rights were provided and explained. A notice of site visit was provided and must remain posted for 30 days.
