During the course of the investigation, LPA conducted observations, received pertinent documents, and interviewed staff. Through the staff interviews and observations that were conducted, it was determined that there are hazardous materials that are accessible to children. LPA observed a storage bin in the outdoor area that is broken and is accessible to children in care. Through the staff interviews conducted, it was determined that there was a child that was left unattended in the outdoor yard for approximately 3 minutes.
