On 07/16/2026, Licensing Program Analyst (LPA) Jennifer "Jen" Beehler conducted an unannounced Complaint Investigation. Upon arrival, LPA was greeted by the Director and provided access to the facility. LPA stated the reason for the visit, toured the facility and collected the census.

There were 60 preschool children and eight (8) staff (6 - Teachers/ 2 - Directors) present which is compliant with ratio and capacity requirements. This agency has investigated the complaint: Staff did not prevent a day-care child from sustaining an unexplained injury while in care. LPA conducted confidential interviews, observed the facility and collected documentation. Based on the evidence gathered, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
