Licensing Program Analysts (LPA) Dung Mac met with Phyu Phyu Shwe, Licensee, for an unannounced inspection due to non-payment of fees. No day care children were present in the home during today's inspection. Phyu Phyu states that she has not had any daycare children enrolled and is no longer operating since March 2019. Phyu Phyu provided LPA with a written letter indicating that she is closing her daycare effective today, January 11, 2022. A daycare license was also returned to LPA during today's inspection.

Forfeiture Letter was given to Licensee during today's inspection. LPA advised Phyu Phyu that her daycare license will be closed effective today, January 11, 2022.
