Licensing Program Analyst (LPA) Kate Huang conducted an unannounced complaint visit to deliver investigation findings regarding the above allegation. LPA met with Facility Representative Jamie Ferguson and explained the nature of today’s inspection.

It was alleged that staff missed feeding one milk bottle to an infant (C1).

During the course of the investigation, LPA conducted facility inspections on 06/03/2026, 06/16/2026 and 07/14/2026; interviewed staff members, the director, and the site supervisor; and reviewed C1’s daily activity report. All individuals interviewed acknowledged that on 05/27/2026, C1 was not given milk after her first bottle at 8:00 a.m. until her second bottle at 4:29 p.m. Interviews revealed that the staff responsible for C1 forgot to check C1’s feeding schedule. The daily activity report for C1 indicated she received her first bottle at 8:00 a.m. and her second bottle at 4:29 p.m., and that C1 was fed puree twice between those times. Additionally, C1’s Needs and Services Plan indicates that she needs to be fed every 4 hours.
