32 | During today’s investigation, LPA observed a child (C3), who is under the age of 12 months old, present in care at the facility. The facility is licensed to serve infants ages 12 months to 24 months old. Director self-admitted that child (C3) has been in care since July 14, 2026, and that the child’s parents or authorized representatives are not always present with the child in the classroom. LPA reviewed the Infant Sleep Observation Form for child (C3), which documented attendance beginning July 14, 2026.
Based on LPA observations, record reviews, interviews and evidence gathered which were conducted, the preponderance of evidence standard has been met, therefore the above allegation, Licensee is operating beyond the conditions specified on the license, is found to be SUBSTANTIATED.
During today's investigation, a Type B deficiency is issued on the attached 9099-D. Appeal rights were provided.
Exit interview was conducted and the report was reviewed with Director, Nikki Hufana.
A notice of site visit was given and must remain posted for 30 days. |
