32 | LPA interviewed the Regional Director on 06/23/2026 and 07/08/2026. The Regional Director stated that the pest issue was first identified in the playground during the week of April 27, 2026. Although pest control treatments were conducted on May 19 and June 17, 2026, the flea problem remained ongoing. On July 2, the pest control vendor notified the center that fleas were still present on campus and that additional treatment was required. The center continued to have children in care until its last day of operation, 07/08/2026. LPA also reviewed documentation showing proof of the two pest control treatment services.
Based on the evidence gathered, the preponderance of evidence standard has been met and therefore the above allegation is Substantiated.
One TYPE B deficiency was cited today with civil penalty as a result of the investigation. The report was reviewed and discussed with the facility representative, Irma Pareja Reina. Appeal rights were provided. |
