13 | On 06/11/2026, Licensing Program Analyst (LPA) Deanna Villagrana met with Assistant Director Shirin Malektojar to deliver findings for the above allegation. LPA explained the nature of the visit. LPA observed 11 infants in room 2 with two teachers and one aide. An additional child arrived during the walk through. LPA observed 15 preschool children with two teachers on the preschool playground.
Based on observation, interviews conducted and pertinent documentation, although the allegation of staff left child in a soiled diaper resulting in rash may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated.
No deficiency was cited.
A notice of site visit was given to Shirin Malektojar and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Appeal rights were provided. |
