On 9/22/26 @ 3:17pm, Licensing Program Analyst (LPA) Erica Laird conducted an unannounced complaint inspection, and met with director Mindy Ulsh. It was alleged that staff inappropriately discipline daycare child; specifically staff (S1-S2) made a child sit on timeout for an extended period of time.

On 8/18/26 LPA Laird conducted an interview with director Mindy Ulsh. Mindy stated she had no knowledge of the allegation. Mindy stated she didn't know of (S1-S2) inappropriately discaplining a child in care. Mindy stated she had no knowledge of staff having a child sit on timeout for an extended period of time.

On 8/18/26 and 9/18/26 LPA Laird conducted interviews with two staff (S1-S2). Both staff stated they had a child (C1) sit with them for approximately 20-45 minutes because C1 kept trying to wander off during a field trip.

On 9/22/26 LPA Laird conducted an interview at the facility with two children (C1-C2). Both children stated C1 had to sit on timeout during a field trip. Both children stated C1 sat on time out for approximately 5 minutes. C1 stated if they tried to kept up the teachers said, "no" and made them sit there.

On 9/18/26 LPA Laird conducted an interview with one parent (P1). P1 stated they had no knowledge of the allegation.

Based on statements made by staff that they had a child sit at a table for an extended period of time, LPA Laird determined there is sufficient evidence to suggest the allegation occurred.

Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D.

LPA Erica Laird informed director, Mindy Ulsh that this report dated 9/22/26 documents 1 Type A citation(s) which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Erica Laird informed the Mindy Ulsh to provide a copy of this licensing report dated 9/22/26 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with director Mindy Ulsh. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

On 9/22/26 LPA Laird conducted an interview with two children (C1-C2). One child stated teachers yell at children in care. One child stated teachers do not yell at children in care.

On 9/18/26 LPA Laird conducted one parent interview (P1). P1 stated they have heard from one of their children that teachers yell and have heard from another child that teachers do not yell.

On 8/18/26 and 9/22/26 LPA Laird observed staff interacting with children in a positive manner. LPA Laird did not observe staff yelling at children in care. LPA Laird observed S1 redirecting children.

Based on interviews and observations, LPA Laird has determined there is not sufficient evidence at this time to determine if the allegation occurred.

Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated.

Exit interview conducted and report was reviewed with director Mindy Ulsh. Appeal rights were provided.
