LPA Mendez interviewed staff (S1) on 5/12/22 at 1:29pm, staff stated that a child (C1) had tripped a few times, C1 had tripped over the curb of the playground and C1 hit their forehead. LPA Mendez interviewed parent (P1) on 4/15/22 and P1 stated that they were not informed when their child received injuries. P1 stated that when they had picked up C1 and noticed C1 had a bump on their head and had to ask licensee what happened during pick up.
LPA Mendez had observed that licensee did communicate with P1 regarding C1 sustaining injuries through an exchange of text messages.
Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated. An exit interview was conducted. The Notice of Site Visit must be posted for 30 days.
