32 | Five staff (S1-S5) were interviewed on 4/9/2026, 7/21/2026 and 8/5/2026. Staff stated that when a child is injured at the facility, the staff communicates with parents/authorized representatives via written report, ProCare (text messaging system) or verbally when the child is picked up. During staff interviews, all staff stated that children are constantly supervised, that staff will walk around the facility to ensure that they can monitor children during various activities at all times and that there are always staff to ensure that ratios are met.
Five parents (P1-P5) were interviewed on 7/17/2026 and 7/22/2026. All five parents interviewed stated that if their children were hurt while in care, they were always notified by the facility via a report. One parent interviewed stated that there appears to be too many children for staff to supervise but did not indicate how many children or staff are present. Four of the five parents interviewed stated that they had no concerns about the amount of supervision provided to children in care.
On 7/21/2026, LPA Curiel reviewed eleven children’s records and observed completed injury reports in the files which included a description of what happened, actions taken by staff, where the incident happened, what time it occurred and who witnessed the event. LPA observed incident reports signed by teachers, staff and the director. Admission agreement, handbook and bite policy were provided by the facility and reviewed. During inspections conducted on 4/9/2026, 7/21/2026 and 8/5/2026, LPA observed staff adequately providing supervision to children in care and operating within their licensed capacity and ratio requirements.
Based on the evidence obtained from interviews, records review, and observations, it could not be determined that injuries sustained by children in care were not reported to the children’s parents/authorized representatives or that staff were not adequately supervising children. Although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred, and the findings are unsubstantiated.
An exit interview was conducted, and the report was reviewed with the Director Lisa Erlandsson-Ward. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
