LPA, Glenn Ouye, previously met with Licensee on 02/03/2025 to open the complaint and obtain records. Licensee recently relocated and obtained a license under facility number 483010873. This complaint was investigated by the Department’s Investigative Branch’s Investigator, Fizza Shahzad.
During the complaint investigation, records were reviewed and interviews conducted with LS, staff, parents, and children from 02/28/2025 through 07/14/2025. Investigator Shahzad accompanied local law enforcement in an interview of the involved child (C1) regarding allegations of inappropriate touching and hitting. LS denied the allegation stating that she never touched C1’s private part or hit them on the buttocks and that staff was always around. LS advised that the changing table was in an open area and she never assisted C1 by herself.
