32 | The Director explained that the facility uses an in-house KinderCare application to communicate with authorized representatives and document children's daily activities, including meals, diaper changes, and daily reports. According to the Director, facility policy requires staff to document each diaper change in the application at the time it is completed. The Director acknowledged, however, that staff do not always enter diaper changes into the application immediately when classroom demands require their attention. The Director stated that, during periods of increased activity, staff prioritize the care and supervision of children before completing electronic documentation. As a result, the timing reflected in the application may not accurately correspond to the actual time a diaper change occurred and may indicate longer intervals between diaper changes than actually occurred. According to the Director, staff complete the documentation as soon as practicable after attending to the needs of the children.
The Director stated that it is the facility's policy to notify a child's authorized representative whenever a child sustains an injury or is involved in an unusual incident while in care. The Director explained that staff are required to complete a written incident report documenting the nature of the injury or incident, the circumstances surrounding the event, any care provided, and the notification made to the child's authorized representative. According to the Director, incident reports are reviewed with the child's authorized representative at the time of pick-up, signed by the authorized representative when applicable, and maintained in the child's file.
The Director further stated that if a staff member observes visible scratches, marks, or other injuries on a child while the child is in care, staff are expected to assess the injury, document the incident as appropriate, and notify the child's authorized representative. The Director explained that injuries or marks located on areas of the body covered by clothing may not be readily observable during routine care and supervision. According to the Director, unless such marks are discovered during activities such as diaper changes or clothing changes, staff may not be aware of their presence and therefore may not have an opportunity to report them to the child's authorized representative.
