13 | Licensing Program Analyst (LPA), Ivet Zamora Perez and Licensing Program Manager (LPM), Melchisdeck Augustin made a subsequent complaint investigation visit and met with Licensee (LS), Diana Cuevas, to deliver the finding regarding the above allegation. LPA Rosales-Meza met with LS on 03/16/26 to open the complaint. It was alleged that the Licensee allowed an adult (A3) in the home who presents a risk to children in care.
The Department Investigations Bureau (IB) obtained law enforcement (PD) reports and document(s) from other external agencies. LS, staff (S1), children (C1-C3), and adults (A1-A3) were interviewed, which noted that A3 did not reside in the home, and A3 did not visit the home during the facility’s operating hours. According to LS, she and her staff were present to supervise the children, A3 visited the home one or two times a month when the facility was already closed or on the weekends, and A3 did not stay the night. Based on accounts provided by S1, C1-C3, and A1-A3, confirmed they had not seen/witnessed A3 visit the facility during the facility’s hours of operation. On 08/14/23, the Department’s Legal Division issued a Decision and Order which excluded A3 from working or residing in all licensed facilities, effective 08/24/23. Conitnue to LIC 9099-C
