25 | On 07/07/2026, Licensing Program Analyst (LPA) Valerie Mireles met with Director Sonja Stephens for an unannounced Case Management inspection. A complete file review was conducted prior to today's inspection. LPA toured the facility inside and outside and a census was taken. There were 38 children separated into three classrooms. The purpose of today's inspection was to address an unusual incident that took place at the facility on 06/29/2026.
An Unusual Incident Report was submitted to the Fresno Community Care Licensing (CCL) Office regarding an incident that occurred, on 06/29/2026, involving a daycare child (Child #1) that tripped and fell onto the corner of a table while walking resulting in the child sustaining an injury to the child’s right eye. On 07/07/2026, Director Stephens reported that as the children were coming in from outside activities, Child #1 was walking over to wash hands when the child tripped, causing the child to fall and hit their right eye on the corner of a table. Staff immediately provided aid and comfort by applying a cloth and ice to the injury. Parents took the child to seek medical treatment, where the child received five stitches. The child returned to care, on 07/07/2026, without restrictions.
On 07/07/2026, LPA observed the classroom where the incident took place. The carpet and flooring were inspected and were in good condition with no visible defects. There were two teachers and approximately 15 children present transitioning from outdoor play and into the classroom where the incident occurred; therefore, adequate supervision was in place.
Based on the information obtained, LPA determined staff handled the incident correctly and reporting requirements were met. LPA determined facility staff took appropriate measures to address the child’s injury, following proper policies and procedures and no regulations were violated.
Per the California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency was cited during today’s inspection. Exit interview conducted with the Director Sonja Stephens.
