32 | Interviews with S1 and Facility Representative revealed the incident involving Child #01 (C1) was not reported timely to the child’s authorized representative, nor to the Department. LPA discussed Reporting Requirements with Facility Representative and with Assistant Director, who confirmed their understanding that such incidents shall be reported timely both to the Department and child’s authorized representative moving forward.
In regard to Allegation #2, LPA was able to corroborate the allegation through interviews and records review.
Based on interviews and records review, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. A Technical Violation (TV) was issued, and LPA Provided Technical Assistance (TA) to Facility Representative as well for reference.
No deficiencies were cited during today’s inspection.
A notice of site visit was given and must remain posted for 30 days. Appeal Rights were provided to Facility Representative. Exit interview conducted and report was reviewed with Facility Representative Marlene Ybarra.
