A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“The incident report reviewed was missing the foster home's name, address, and phone number along with the child's full name, age, gender, and date of admission.”
“It was determined that the child's injury occurred on 7/30/24, the child was taken to the ER on 8/5/24, and the serious incident report was signed and dated on 8/15/2024. This is past the 24-hour requirement of reporting and documenting critical injuries that require medical treatment.”
