Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Per children record review, LPA observed four children’s record missing and incomplete without parent or parent representative signatures.”
“Per staff record review, LPAs Wallin and [name withheld] observed licensee needs mandated reporter training for licensee and (2) assistants.”
“Per staff record review, LPA's Wallin and [name withheld] observed licensee missing TB immunization and two staff missing immunization's for measles, pertussis, influenza and TB clearance.”
