Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on LPAs' observations there was clutter, spoiled fruit with gnats, scissors, construction materials, and trash throughout the dining room, that could be accessible to children. This poses a potential risk to children in care.”
“Based on LPA’s interviews, staff #1, Staff #2, and Staff #3 disclosed that S1 has been out of the country off and on since October of 2023 until present. Licensee is out of the facility more that than 20% of the time.”
